What to bring with you to your appointment
- Please bring a listing of any medications that you are taking, both prescribed and over-the-counter, along with the dosage amount. (Ask one of our Medical Assistants for a handy wallet card for listing your medications.)
- Also include any forms that were sent to you in the mail or handed to you at your previous appointment.
- We will need to see your insurance card and photo identification each time you visit us to assist in billing and comply with federal laws.
Parking
- 885 Mission St – parking is available under and behind the building. You can access this lot from Mission Street. Please be aware that parking at the park across the street at Bush Park may result in a ticket from the city.
- 875 Oak St - Center for Outpatient Medicine (COM) parking is available by proceeding under the building from an entrance on Winter Street. There is also parking in the structure on the corner of Mission and Capitol Streets or under Salem Hospital's Building A.
